Oakland Community Health Network taking service delivery survey

Each year, Oakland Community Health Network (OCHN) issues surveys seeking input to help draft its Strategic Plan (three year) or Annual Plan and Budget (one year). Surveys are sent to people receiving services; family members and advocates; and community agencies and their staff. The surveys are also posted on OCHN's website. Additional data is gathered from other venues, such as town hall meetings or visits to community service sites.

Both the Strategic and Annual Plan are used by OCHN to ensure people receive valuable, evidenced based and trauma informed services. The plans include numerous goals and objectives designed to assist people who receive public mental health services in attaining their life aspirations. Residents' support of this important planning process by completing OCHN's FY19 Service Delivery Feedback survey is greatly appreciated.

A community forum to discuss the FY19 to FY21 Strategic Plan and survey results is being held on Tuesday, June 26, from 4 to 6 p.m. The community forum for the FY19 Annual Plan and Budget is scheduled for Tuesday, August 28, from 4 to 6 p.m. Both are to be held at OCHN, 5505 Corporate Dr. in Troy.
