The Michigan Department of Health and Human Services (MDHHS) has issued?a?Grant?Funding Opportunity?(GFO)?for nonprofits to provide community navigators to help residents?impacted by new federal requirements through H.R. 1, as they?apply for or renew Healthy Michigan Plan (HMP) coverage or?Supplemental Nutrition Assistance Program (SNAP)?benefits.
The focus of the initiative is to strengthen statewide HMP and SNAP navigation, outreach and eligibility support capacity by leveraging health and human service providers, nonprofit organizations, other community-based navigation partners and a flexible navigator workforce. Successful applicants will establish a coordinated statewide navigation network by conducting a competitive bid process to identify secondary partner organizations to serve as community navigators. ??
Eligible applicants must be?501(c)(3)?nonprofit organizations.? ?
The initial implementation period is anticipated to run through Saturday, Sept. 30, 2028, and activities may be extended through Monday, Sept. 30, 2030. There is about $12.25 million available, and MDHHS expects to issue one or more awards. Project?director requests?to access the application?are due by 5 p.m.,?Thursday, Sept. 24. Grant applications?must be?submitted electronically through the EGrAMS system by 3 p.m., Wednesday, Sept. 30. ?
A pre-application conference will be held to provide instruction on using the EGrAMS system.?
The pre-application conference will be held?at?10:30 a.m.,?Wednesday, Sept. 16, and will last approximately 90 minutes.
